THE FOLLOWING DOCUMENTS ARE REQUIRED TO ENROLL:

1. Up-To-Date Immunization Record
2. State Issued Birth Certificate
3. Two UTILITY or LEASE/LOAN documents showing proof of residency. (I.E. Water, electric, sewage or mortgage payment) Drivers licenses, cut off notices, and junk mail are NOT accepted.
4. KINDERGARTEN STUDENTS are required to provide documentation of a passed vision screening within the previous twelve months.
5. Native American Students need to present their CDIB and Tribal Cards or parents cards.
6. PK students MUST have been born on or before 09/01/18.  
7. K students MUST have been born on or before 09/01/17.
